The revision rhinoplasty procedure tends to take longer than primary rhinoplasty. At our Atlanta-area practice, revision nose surgery can take from one to three hours to perform. The revision rhinoplasty procedure takes longer because of the need to correct problems associated with the initial surgery. Incisions may be confined to the inside of the nose (closed rhinoplasty) or may involve an additional small incision under the tip of the nose (open rhinoplasty). There are times when open rhinoplasty is preferred because it can offer an overall better result.
Once the incisions are made, the skin of the nose is elevated off the underlying bone and cartilage. What occurs next depends on the patient's particular needs. Cartilage and bone may be repositioned or missing structures may be replaced. When the desired result is achieved, the skin is re-draped over the new nose framework and the incisions are closed. A splint is applied to the outside of the nose to help retain the new shape while the nose heals. Soft, absorbent material may be used inside the nose to maintain stability along the septum, the dividing wall between the air passages.
